Welcome to the Coalition to Back Black Businesses (CBBB) $5,000 grant application. Before beginning your application, we encourage you to review all of the eligibility criteria for the program to make certain that you qualify to apply. Here are a few of the criteria that your business must meet:  

• Employ between three (3) and 20 individuals, including yourself, any full- or part-time employees, and consultants/contractors working for your business under a 1099 agreement  
• Be a for-profit company. Non-profit organizations are not eligible to apply  
• Have a valid employer identification number (EIN). Businesses that are run with a social security number (SSN) are not eligible to apply 

Please fill out the application in its entirety. In addition to completing the application questions, you will also need to upload a completed IRS W-9 form for your business. In the form, you must fill in the following information:

• Business name
• Business address
• Employer identification number (EIN) - "Part I" of the form
• Sign and date the form - "Part II" of the form

You can save your progress and return to your application at a later date if necessary. Once you submit your application, however, it is considered final and you will not be able to make any changes to it. 

Kindly complete your application by the deadline indicated in the email invitation that you received for the grant.

Please note that grants will be awarded based upon review of submitted information. Submitting an application does not guarantee that a grant will be awarded.
